Thank you for choosing Monmouth Eye Care! We are committed to giving our patients the very best in eye care. When planning your visit, please keep at few things in mind:
- Appointments: Routine eye exams can sometimes book a few weeks out, so call us as soon as you can to plan your full exam. New patients and children are welcomed. Emergency appointments will be scheduled promptly.
- Insurance: We accept most medical insurance plans, please check with our receptionist when scheduling your appointment. We do not accept vision plans, such as VSP. Please check with your insurance company if you require referrals to see a specialist, every insurance plan is different.
- To Bring to Office: Please bring your insurance cards, a referral if needed, any eye drops or medications being used, glasses (even over the counter), and contact lens information (if worn). If you’re a new patient, old records are helpful but not necessary.
- Forms: Printing out the office paperwork can save some time, find them here: Patient Forms. If not, please arrive 15 minutes before your appointment to do so.
- Office Hours: Monday-Friday, 8:30am-5:00 pm. Any calls made after 5:00pm can leave a message with our answering service. Once a month on Wednesday , we will offer appointments until 6:00pm. In an emergency, service can contact the doctor.
